Application form KR13 shall be signed by the executive board or the liquidator (if more than one liquidator has been appointed, the application shall be signed by all liquidators).

Additional information:

• the application must specify the time period for creditors to submit claims, as well as the address at which creditors may submit their claims;
• the time period for creditors to submit claims is one month, unless a longer time period for creditors to submit claims has been specified in the resolution of the shareholders’ meeting or the court ruling on termination of the activities of the limited liability company;
• the address specified for submission of creditors’ claims must correspond to a specific address registered in the National Address Register;
• information on the beneficial owner must be provided in the application.
