The application form KR5 shall be signed by the executive board.

Additional information:

• the application must indicate the name, surname, personal identity number of the procuration holder (if the person does not have a personal identity number issued by the Republic of Latvia, the date of birth, the number and date of issue of the identity document, the country and authority that issued the document, and the address at which the person may be reached), and the scope of the rights of representation;
• if the procuration holder has the right to transfer, pledge or encumber immovable property with rights in rem, this must be specifically indicated in the application;

• a joint procuration may be issued to several persons simultaneously. In such a case, the joint procuration shall be issued to at least two persons jointly, who shall have only joint rights of representation.
